going to Phuket later this month with my girlfriend and would like to know if there are any lesbian bars around Patong?

Quite a few of the single women or women in groups rent gigolos. Phuket gigolos are the equivalent of "Kuta Cowboys" that single women seek out in Bali. (Many of these women come from Australia.) But I never read a lot about lesbians seeking lesbian bars. You can check with a ladyboy bar, perhaps they will know.

You have nothing to lose by visiting a beer bar and asking a bar lady to go with you. Money is money and this is off season!

I live in Thailand but I don't know of the les bars in Patong, But I can tell you, if you go to the regular bars early on and ask the Thai girls where's the "TOM" bars, they will be happy to tell you/write it down for the taxi driver. "Tom boy bars" is synonymous with les bars in Thailand.
